Please complete your admissions file by submitting the following:
- Current resume
- You may fax or email this document.
- Official MCAT, GMAT, or GRE scores
- To submit your official MCAT score:
- Login to the Testing History site
- Click on “Create and Print my own official score report,” then click “Create.”
- Click on “Retrieve my Report.”
- Save this webpage.
- Email the webpage as an attachment.
- If you have already earned a Master’s degree or other terminal degree (MD, PhD, DC, etc) from a US-accredited institution, you are waived from this requirement.
- Official undergraduate and graduate transcripts
- Davenport will request and receive your medical/veterinary school undergraduate and graduate transcripts that are on file at your medical school. To allow DU access to your records:
If the most current copies of your official transcripts are not available, please request them. **
- Two professional recommendations
- Davenport will request and receive your letters of recommendation on file at your school.
- If the medical/veterinary school does not have letters of recommendation on file for you, you may use our professional recommendation form.
** Official transcripts must be sent directly from your previous institution to Davenport.

Davenport will also accept faxed official transcripts as long as the transcript is faxed with a cover sheet from the school’s Registrar. These transcripts may be faxed to Davenport’s Registrar at 616-732-1150.
