A medical/veterinary school employee

Please complete your admissions file by submitting the following:

• Current resume
  • You may fax or email this document.
• Official GMAT or GRE scores
  • If you have already earned a Master’s degree or other terminal degree (MD, PhD, DC, etc) from a US-accredited institution, you are waived from this requirement.
• Official undergraduate and graduate transcripts
  • Official transcripts must be sent directly from your previous institution to Davenport. 
  • Transcripts may be mailed to:

    Grand Rapids, MI 49503
  • Davenport will also accept faxed official transcript as long as the transcript is faxed with a cover sheet from the school’s Registrar.  These transcripts may be faxed to Davenport’s Registrar at 616-732-1150.
• Two professional letters of recommendation
  • You may use our professional recommendation form.
