Davenport University's Upsilon Phi Delta Student Application Form

Mission Statement
The Upsilon Phi Delta Honor Society is for students who are enrolled in the College of Health Professions Health Services Administration (BS) Program.

Its mission is to recognize, reward, and encourage academic excellence in the study of healthcare management and policy.

Davenport University's Charter for Upsilon Phi Delta Honor Society Chapter:
Upsilon Phi Delta (UPD) was formed to further the professional and competence of it's members who pursue a career in health administration. Upsilon Phi Delta is administered by the Association of University Programs in Health Administration. The name is derived from Greek words for health, care, and administration. 

The purpose of Davenport University's Upsilon Phi Delta Honor Society is to enhance the professional development and competence of individual members in and for the profession of healthcare administration and management. 

The purpose is achieved by:

  • Recognizing students who achieve distinction in Health Services Administration studies
  • Motivating academic excellence in students studying healthcare management and administration.
  • Elevating high professional standards, ideals, competence and ethics of students in healthcare management and leadership
  • Providing networking opportunities to UPD members for interaction and mutual support
  • Offering financial assistance through scholarships offered by the UPD National Honor Society

Qualifications for Undergraduate Eligible Membership:

  • Declaration of Health Services Administration as major area of study
  • GPA of 3.25 or greater
  • Completed at least 18 credit hours of study in the major. 

As a student of the Health Services Administration Program, I accept the nomination to become a member of the Davenport University's Upsilon Phi Delta Honor Society.

There is a $25.00 membership fee to be submitted with the application. The student will receive a certificate of membership, UPD Pin and Honor Cords to wear at the Graduation Commencement.  To continue, and process this fee please submit the form.
